Initial Patient Instruct.
PREPARE ALLERGENS UP TO TWO WEEKS
IN ADVANCE...USING ADVANCED IQ-ULTRA®

IMPORTANT INFORMATION:

1.
Do not prefill aqueous solutions in advance. To avoid evaporation, fill the aqueous allergens in their proper chamber just prior to applying the chambers to the patient. Make certain that you leave the chamber requiring a liquid allergen empty while prefilling the petrolatum allergens.
2
For easier handling and better control, place completed prefilled series in separate plastic bags and store refrigerated up to two weeks. (For example, if your series has 50 allergens you would use 5 units of 10 chambers each (one unit in each of the 5 wells on the application plate)...making certain you mark each unit “Std.-1, 2, etc.” and that you number the WELL #1 chambers 1,5 - 6,10; WELL #2 - 11,15; 16-20, WELL #3 - 21,25 - 26,30; etc. on the appropriate unit. Note: the number “1" on each WELL on the application plate will correspond to allergen 1, 11, 21, 31 and 41etc. of the series.)

(SEE DIAGRAMS AT THE BOTTOM OF THE PAGE)
1
For optimal work position, place the application plate flat on the table with the five wells at the bottom. (See diagram Application plate)
2
Insert 1 empty IQ-ULTRA chamber/plastic unit (10 chambers) into each of the 5 wells with the tape facing up. The end of the unit with the center notch should be inserted first leaving the two tabs at the bottom. (See diagram A1-2)
3
With a ball point/pencil mark the series name on the tape. Also mark the number of the chambers on each unit (for example: on the first unit in the first well you can mark “1" beside the first chamber location and the number “10" beside the tenth chamber location, and “11" and “20" on the second unit, etc.) This ensures the correct units are placed together for storage and applied correctly on the patient's back.
4

Take a firm grip on both the left and right tab/tape piece and bend back and forth so each piece will break along the perforation. (See diagram A-3)

5
Grip the left and right hand tabs and pull forward keeping the tape and chambers as parallel to the board surface as possible to avoid the board slipping along the table. (See diagram A-3)
6
.(A) Starting with the left hand chamber unit, commence filling the “petrolatum” allergens in the chamber beside the number “1" on the tape and proceed through “5", (skipping any aqueous allergens)....then go to the numbers “6" through “10". Roll the tape with the filled chambers back onto the plastic cover. You have completed unit l. Complete additional units for a series in the same manner. (See diagram A-4) OR
(B)To save time, If you are filling multiple units of the same series, you would commence filling the number “1" chamber on all five units with the same allergen. Then go to “2" and fill with the second allergen of the series in all 5 units, etc. When you finished the 10th allergen in the 10th chamber you would have 5 units of the first 10 allergens of the series completed and marked. Roll the tape/filled chambers onto the plastic covers. (See diagram A-4)
7
Make certain to store, under refrigeration, the prepared units in plastic bags and labeled according to series and numbers to ensure proper control. (For example, all units, e.g. 1 - 10, of the same series in the same bag; OR, all allergens of a complete series can be placed into one bag; OR place those allergens prepared for a specific patient in a marked “patient bag”.)
8
IQ-ULTRA comes with a filter paper in each chamber. When filling the aqueous allergens on the day of use, apply a drop of the solution in each chamber. The amount of solution should be just enough to moisten the filter paper (about 20 :).
