FILLING THE CHAMBERS  
  A. Using table top:
1- Separate the perforated corners of the plastic cover by bending them. 2- Gently pull back the tape until all chambers are available for filling. The tape should not be detached from the cover. (See diagram A-3) 3- Place IQ Ultra units flat on a table top for filling..4- Fill the chambers with allergens so that when applied to the back of the patient allergens no 1, 11, 21,etc... are in the upper left hand chamber of each unit.
 

B. Using IQ ULTRA ® Application Plate:

 
 
The IQ-ULTRA Application Plate simplifies the advance filling of allergens. Specifically designed for the IQ-ULTRA®, the application plate is cost effective and saves nurses/technicians time, allowing preparation of test series up to two weeks prior to use. (Fill petrolatum allergens only, fill aqueous on the day of application).
1- For optimal work position, place the application plate flat on the table with the five wells at the bottom. (See diagram- AP) 2-Insert 1 empty IQ-ULTRA unit into each of the 5 wells with the tape facing up. The end of the unit with the center V cut notch should be inserted first leaving the two tabs at the bottom. (See diagram A-2) 3-Take a firm grip on either of the perforated corners of the plastic cover and separate them by bending them until they break. (see diagram A-2)
4- Gently pull the tape back until all chambers are available for filling. The tape should not be detached from the cover. 5-Starting with the left hand chamber unit, commence filling the "petrolatum" allergens in the chamber beside the number "1" on the Application plate's numbering strip. Proceed through "5", (skipping any aqueous allergens)....then go to the numbers "6" through "10". When you have completed filling of the unit(s) replace the filled tape back onto the plastic cover. Complete additional units for a series in the same manner. (See diagram A-4). 6- Place the completed units into plastic bags, label them (by patient name or series) and store them under refrigeration until the day of application to patient ( fill the aqueous chambers).
 
  IQ-ULTRA® comes with a filter paper in each chamber. When filling the aqueous allergens on the day of use, apply a drop of the solution (about 20 :l) in each chamber, enough to moisten the filter paper.  

PATIENT APPLICATION

 
 

Whenever possible, apply the IQ ULTRA chambers onto the upper part of the patient's back, avoid midline and the scapula. In some cases the upper arms may also be used. If the patient's skin is oily, clean it gently with ethanol prior to application.

 

1- Instruct the patient to bend slightly forward.2- Grip the end of the tape and the plastic cover (diagram A-5). Press the unit onto the patient's back starting from the bottom to top. Ensure that the tape's V cut notches are facing up (diagram A-6). 3- Remove the plastic cover. (diagram A-6)
4- Press the tape with the palm of your hand for about 5 seconds (diagram A-6). The pressure and heat will enhance adhesion. 5- Mark the four corners of each unit with Chemotechnique Skin Markers to indicate each units location on the patient's back. 6- Using the Chemotechnique Record Forms, mark the allergens and corresponding chamber numbers. 7- Provide the patient a copy of “Initial Patient Instruction Sheet”, covering procedural information, do's and don'ts, care and cautions. (Available on our website).

 

REMOVING IQ ULTRA UNITS

 
 

1- After 48 hours remove the patch test units from the patient's back in a rapid diagonal downward motion. This will minimize patient discomfort, and reduce the possibility of chamber detachments. 2- The initial visual chamber imprint on the patient's back indicate excellent occlusion.

 

READING THE RESULTS

 
 

1- Allow about 20 to 30 minutes for the transient skin irritation and visual chamber imprints to subside before interpreting the test results. 2- Reading is normally done at: 48, 72, and in some cases, at 96 hour post application.3- Use the IQ ULTRA Reading Plate to facilitate the reading.
4- The following criteria is used for interpreting the test results:

 
 


- Negative reaction
? Doubtful reaction
+ Weak reaction (non-vesicular)
++ Strong reaction ( oedematous or vesicular)
+++ Extreme reaction (ulcerative or bullous)
IR Irritant reaction
